چکیده

Major toxicities More common: Nausea, vomiting, fever, headache, diarrhea, rash, and anorexia. Less common (more severe): Approximately 5% of adults and children receiving ABC develop a potentially fatal hypersensitivity reaction. Symptoms include fever, fatigue , malaise, nausea, vomiting, diarrhea, and abdominal pain or respiratory symptoms such as shortness of breath. Physical findings include lymphadenopathy, ulceration of mucous membranes, and maculopapular or ur ticarial skin rash. The hypersensitivity reaction can occur without a rash. Laboratory abnormalities include elevated liver function tests, elevated creatine phosphokinase, elevated creatinine, and lymphopenia. This reaction generally occurs in the first six weeks of therapy. Patients suspected of having a hypersensitivity reaction should have ABC stopped and not restar ted since hypotension and death have occurred upon rechallange. Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases have been repor ted. Rare: Pancreatitis, increased liver enzymes, elevated blood glucose , elevated tr iglycerides, and fatigue.

Off-Target Effects of Drugs that Disrupt Human Mitochondrial DNA Maintenance

Nucleoside reverse transcriptase inhibitors (NRTIs) were the first drugs used to treat human immunodeficiency virus (HIV) the cause of acquired immunodeficiency syndrome. Development of severe mitochondrial toxicity has been well documented in patients infected with HIV and administered NRTIs. In vitro biochemical experiments have demonstrated that the replicative mitochondrial DNA (mtDNA) poly...

Nucleotide Reverse Transcriptase Inhibitors

4 Optimal HIV-1 suppression often is difficult to achieve and maintain with currently available antiretroviral drugs, and viral resistance eventually emerges to many drugs. Chronic antiretroviral therapy is also associated with complications such as body fat redistribution, hypertriglyceridemia and hypercholesterolemia, abnormal glucose metabolism, lactic acidosis, pancreatitis, avascular necro...

Bench-to-bedside review: Severe lactic acidosis in HIV patients treated with nucleoside analogue reverse transcriptase inhibitors

Nucleoside reverse transcriptase inhibitors (NRTIs) are effective antiretroviral therapy for the treatment of HIV-infected patients. NRTIs can induce mitochondrial impairment that leads to a number of adverse events, including symptomatic lactic acidosis. In the present review, we describe the underlying mechanism of NRTI-induced toxicity and the main clinical features of this infrequent, but s...

Nucleoside analogue reverse transcriptase inhibitor options: a re-examination of the class.

The main options for dual nucleoside (or nucleotide) analogue reverse transcriptase inhibitors (nRTIs) as a component of initial antiretroviral therapy regimens are tenofovir/emtricitabine, zidovudine/lamivudine, and abacavir/lamivudine as fixed-dose combinations. Resistance to nRTIs can limit usefulness of many of the drugs in the class. Investigation of triple nRTI regimens has shown that zid...

Twenty-six years of anti-HIV drug discovery: where do we stand and where do we go?

The human immunodeficiency virus (HIV) has now been established as the causative agent of the acquired immunodeficiency syndrome (AIDS) for over 20 years. During this time an unprecedented success has been achieved in discovering anti-HIV drugs as reflected by the fact that there are now more drugs approved for the treatment of HIV than for all other viral infections taken together. The current...

Nucleoside Analogue Reverse Transcriptase Inhibitors Differentially Inhibit Human LINE-1 Retrotransposition

BACKGROUND Intact LINE-1 elements are the only retrotransposons encoded by the human genome known to be capable of autonomous replication. Numerous cases of genetic disease have been traced to gene disruptions caused by LINE-1 retrotransposition events in germ-line cells. In addition, genomic instability resulting from LINE-1 retrotransposition in somatic cells has been proposed as a contributi...
